Position Summary:
Patient Transfer Center Associate coordinates the reporting, documentation and review of all incoming admissions and transfers to JMH.
Duties & Responsibilities:
Documents and maintains manual/electronic logs of all incoming requests for transfer. Maintains constant line of communication with Transfer Center Clinical Care Coordinators regarding new transfers and bed assignments. Works closely with the Transfer Center Clinical Care Coordinator to initiate and arrange for patient bed assignment according to policies governing specialties, services and requested accommodations. Performs various fact-finding tasks upon presentation of transfer and collects and verifies necessary financial and or third party information in order to determine credit and collections eligibility. Obtains personal, credit and financial information during initial presentation of transfer process. Records and verifies that all financial information collected is accurate. Checks patient’s status of each new transfer for residence status. Notifies the patient’s insurance company in order to obtain payment authorization. Obtains prior authorization and or letters of guarantee. Communicates frequently with Transfer Center Patient Financial Specialist or Patient Financial Services regarding registration and financial clearance issues. Verifies readiness of assigned beds on the inpatient floors and critical care units. Provides administrative/clerical support to Transfer Center Clinical Care Coordinators (e.g., provide telephone service coverage; provide direction to external facilities regarding transfer request process, etc.) Assesses and determine administrative appropriateness of transfers as necessary. Keeps abreast of regulatory standards (e.g., EMTALA) that influence decisions about establishing transfer priorities. Maintains Transfer Center documentation, patient records and transfer logs as per departmental, regulatory, and organizational standards. Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise). Performs all other related job duties as assigned.
Qualifications
Experience:
Generally requires 0 to 3 years of related experience.
Education:
High School diploma is required.
License Certification:
Medical Assistant, Paramedic or EMT certification preferred.
Knowledge Skill Abilities:
Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with critical thinking and professional composure. Ability to understand and follow instructions. Ability to exercise sound and independent judgment. Knowledge and skill in use of job appropriate technology and software applications.
Physical Demands:
Job function is sedentary in nature and requires sitting for extended periods of time. Function may require frequent standing or walking. Must be able to lift or carry objects weighing up to 20 pounds. Jobs in this group are required to have close visual acuity to perform activities such as: extended use of computers, preparing and analyzing data and analytics, and other components of a typical office environment. Jobs in this group are required to function in a fast paced environment with occasional high pressure or emergent and stressful situations. Function is subject to inside environmental conditions, with occasional outdoor exposures. Possible exposure to various environments such as: communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and medical office environment. May wear Personal Protective Equipment (PPE) such as gloves or a mask when exposed to hospital environment outside of office.
