Description : Iris Healthcare is an Austin-based company offering an advance care planning program by phone and video for people facing serious medical conditions.We are currently seeking licensed, experienced healthcare professionals for part-time facilitator role for 20-30 hours per week during workday hours. Hours are primarily afternoon/evening beginning at 1pm ET/12pm CT/11am MT/ 10am PT Monday-Friday and some Saturday hours/availability after onboarding.This completely REMOTE position offers the flexibility of setting your own schedule within needed timeframes and the convenience of working from home.Learn more about Iris Healthcare at www.irishealthcare.comFACILITATOR:An Iris Plans facilitator is responsible for holding discussions with patients and their loved ones through structured advance care planning. This healthcare expert will provide education on the patient’s primary medical condition and on such medical interventions as feeding tubes, ventilation, and CPR. The facilitator will capture details of the conversation that will be utilized to generate state-specific advance care planning documents.Requirements:Please include a resume and a note on whether or not you have ACP experience.
