Description : We have an exciting opportunity for a band 3 Health care assistant to join the evening service within the Out of Hours team. The post holder will work across Hull and east Riding and will work a 5-hour shift pattern between the hours of 4pm and 9pm. The role will involve working independently to deliver care to patients in their own homes delegated by the shift coordinator who is on duty within these hours. The successful candidate will have a period of probation to work through the clinical skills passport and this will require a period of time within the day service to complete shadow shifts. The contracted hours will be 25 hours, 20 hours or 15 hours per week.
The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal skills and be able to communicate effectively with patients, carers and other members of the MDT.
Additionally, you will need to demonstrate IT literacy and this post requires that you have the ability to travel between locations to carry out your work and to attend training and visits; daily access to transport is therefore essential. The ability to commute within CHCP CIC’s geographical areas with access to appropriate means of transport is essential.
