Description : About The Service
The Community Resource Team (CRT) operates throughout Cardiff. We work 365 days of the year 24 hours per day. We support the citizens of Cardiff preventing unnecessary hospital admissions and facilitate discharges from hospital. We work closely with our health colleagues to support the patient to achieve what matters to them. We carry out an assessment and build a plan to support the individual’s needs.
About the job
Care staff will support the client to achieve their goals, working alongside health professionals. A service delivery plan supports the carer in their daily duties. Carers work to a rota, working AM and PM shifts. Each carer has a mobile device that allocates their daily tasks. This is an important role working to a structured plan and specific goals. The need for good observation and recording skills are essential, a flexible, focused approach is required. Contracted hours will be 30 hours per week.
What We Are Looking For From You
Experience of working with adults or relevant care, good recording and observational skills. The ability to train and achieve the relevant QCF level required for the post. Flexible, and able to work in a dynamic and fast changing service.
