The position provides the most up-to-date education and information on various services and programs to healthcare professionals like physicians, caregivers, and the general community. The Patient Care Coordinator is responsible for strengthening relationships with community partners to ensure consistent referrals, visibility, and program awareness within the community.What you will doBuilds rapport and strengthens existing relationships and cultivates new ones through working with medical, dental, school health coordinators, government health associations, and referral agencies, etc.Markets and promotes services and programs on a scheduled basisCoordinates referral matters and sets up meetings between the NHWS clinical provider and referral sourcesSearches out possible and existing referral sources, contacts them, builds and maintains rapport, and sets up meetings between NHWS provider and the actual referral sourceAssist in creating and implementing branded advertising campaigns and assist with other technical contentWill organize mass mailings, including handling and tracking returns and responsesMaintain a database for referral sources (e.g. physicians, agencies, etc.)Identify, qualify (characterize) and contact prospective referral sources and agencies for follow up meetings.Create monthly marketing reports providing insight into defined Company StatsAssist in the development and implementation of patient satisfaction surveyServe as a single point of contact for new referrals/patientsSchedule new patients for appropriate services, following Clinic guidelines for each disciplineAnswer questions and concerns from patients, community partners, and clinic staffRequired Knowledge, Skills and AbilitiesA genuine interest in and liking for other peopleAbility to build rapport with front office staff or potential referral sourcesConfident and outgoing personality with the willingness to accept the occasional rejectionGood organizational skillsThe ability to adapt and think quicklyAttention to detailJob Type: Full-timePay: Up to $23.00 per hourBenefits:401(k)401(k) matchingDental insuranceEmployee discountHealth insuranceHealth savings accountPaid time offVision insuranceSchedule:8 hour shiftDay shiftMonday to FridaySupplemental Pay:Bonus payCOVID-19 considerations:As medical providers and essential workers we have remained open throughout the COVID-19 Pandemic.
