Description : people is recruiting for a Support Worker to join the team at our Your Home Out Reach Service.Your Home service provides support to people aged 16+ who are transitioningout of homelessness to longer term accommodation or those who are atrisk of becoming homeless.Your Home offers a variety of support, depending on what each person needs. We understand that everyone has had different experiences, and we aim to create relationships based on trust to help build resilience and self-worth.Reporting to the Senior Support Worker, the main duties of the position will include: Arranging and facilitating key work meeting to develop and review support plans in collaboration with the people we support to meet their individual needsDeveloping life skills with the people we support including how to maximise income, budgeting, shopping, cooking, home maintenance & repair and any other skills that aid to independenceAssisting the people we support in their transition from temporary accommodation to permanent housingProviding advice on a range of subjects including housing options and welfare benefitsFull details can be found in the Job & Person Specification in the Application Pack on our websiteHours of workAn average of 35 hours per week. These hours are usually worked Monday to Friday and flexibly within the hours of 8am and 6pm with a one-hour unpaid break. Variations to these hours must be agreed with your line managerBlend of office and home working and in the local communityBenefits IncludeAnnual leave entitlement of 210 hours holiday (equivalent to 6 weeks) per year in the first year rising to 280 hours (equivalent to 8 weeks) per year in the second.
