Description : General Summary:
The individual applying for this position must be dependable, thorough with communication and documentation, must be able to multi-task, easily gets along well with others (surgeons, patients and co-workers) and be able to problem solve. The Care Coordinator performs a variety of duties that are typically 100% clerical/telephonic in nature. There is no in-person/face-to-face communication with patients at this time. This position requires excellent communication skills in planning and coordinating day to day office activities. The individual must work independently while taking ownership and be accountable for their specific job duties.

COMMUNICATION- Excellent interpersonal skills and respectful attitude with surgeons, co-workers and patients is a must. This Care Coordinator is responsible for making multiple patient follow-up phone calls daily. Communicates with Care Coordinators/Case Management team any information received from physician, patient or other employees about the patient’s care or needs. Prepares and mails packets to patients daily. Communicates clearly and professionally with physicians and other staff via email and phone, documents clearly, notifies appropriate Care Coordinator or Case Manager appropriately and adjusts schedules accordingly as indicated. Files all office records as necessary.
DOCUMENTATION- Thorough and accurate documentation is essential for continuity of care. The Care Coordinator is responsible for the review of daily reports and inputting patient demographics into computer software system. Responsible for review of new scheduled surgeries and enters the information into the case management database. This position makes phone calls to patients and does a thorough patient chart review of relevant and most recent notes from other Population Care Coordinators or Case Managers before making patient calls to ensure continuity of care.
