Description : Purpose…
YOU will support CARERS during inpatient periods using your empathy, knowledge and/or professional or lived experience of caring and health and social care.
Carers registered with our Charity will rely on YOUR guidance and a listening ear, YOUR wellbeing support, organisation of peer support opportunities, help with planning for the future related to their care responsibilities and introduction to preventative services.
YOU will also raise awareness of the challenges that unpaid carers face and provide access to appropriate support for carers within West Sussex by liaising with health and social care professionals within the acute and community hospitals as part of the multi-disciplinary team (MDT).
The carers we support come from such different walks of life that we are particularly interested in attracting candidates from similarly diverse backgrounds, including Asian, Arab, Black, Mixed/Multiple Ethnic Groups, White Other (e.g. Eastern European, Gypsy, Roma) and any other Ethnic minorities. ABOUT YOU
You are looking for a meaningful job that will make a difference to your local community.
You enjoy connecting with people on a deeper level and can work independently and manage a team cross county within acute and community hospitals.
You have a good command of English and a high level of empathy when discussing sensitive subjects.
You have had exposure to / experience with / interest in social care and health, particularly as it relates to working with carers in a hospital setting.
You are confident using technology, such as chat applications, databases or CRMs, or are willing to learn.
You can be flexible about the way you work, i.e. can travel across West Sussex if needed, work from East Surrey/Princess Royal and Royal County Brighton/St Richards Hospitals, or do some work remotely.
You have the UK right to work and are prepared to undertake relevant background checks, including references and Enhanced DBS (working with adults).

KEY DUTIES
YOU will line manage a team of 5 and be responsible for 1-2-1’s, training and supporting them in their roles.
YOU will have triage responsibilities and will support the hospital manager in partnership working.
YOU will provide information, guidance and support to adults and young people who look after a relative or friend who could not manage without their help during a period in hospital.
YOU will manage a team of staff and volunteers and will provide mentoring to volunteers learning about our carer services.
YOU will know of/learn about the Think Carer principles and promote them across all networks, including best practice knowledge, understanding of local/national carer landscape and raising awareness of carer issues.
YOU will partner with local health, social care and voluntary sector organisations to enable carers to access timely and appropriate support.
YOU will record/report using our outcome-based tools and systems to evidence the work we do for the community and identify any areas for improvement.
YOU will support the Hospital Team Manager and support with the development of our Hospital services
YOU will ensure safeguarding vulnerable adults and children and the health and safety of staff, volunteers and carers is considered.
