She takes a real pride in her flat which she calls her ‘home’.”We have an exciting and rewarding opportunity for a Senior Support Practitoner to join our friendly and welcoming team working at our Service Amadeus House in Ealing. You will work as part of a small team in a home from home environment where no two days will be the same.Our service Amadeus Recovery House is a 17 bedded community based short stay crisis house. We offer a short term intervention for people experiencing a mental health crisis as an alternative to a hospital stay. We empower people to develop their own coping skills, build their resilience and take control of their lives, enabling them to return home as soon as possible and stay well once home.How will you make a difference?Support the Team to process referrals and lead on this task when necessary.Support individuals, with a range of social needs, that may impact on their move on from the service, by engaging with specialist services.Act as a resource to others, providing accurate, appropriate information, advice and support with support planning to the multidisciplinary team, service users and their families or carers.Ensure a person-centred approach which treats individuals with dignity and respect, and meets their diverse or unique needs and secures the best outcomes possible.signpost in relation to welfare & benefit issues with sound working knowledge of PIP, Universal Credit and ESAFormulate support plans in partnership with the individual, associated carers/family members, the MDT and other key stakeholdersPromote the involvement of service users and their carers at all stages of the discharge planning process, decision making relating to discharge, kept informed of the move on plan, timescales, and any barriers to move onSupport the Managers and take the lead in their absence, supporting staff and coordinating the shift by ensuring that incident reports are made and issues escalated to appropriate persons.Liaise with the bed managers to ensure an effective and efficient flow of admissions and discharges in and out of the service.Could this be you?You will have strong communication skills and a understanding of mental illness. But aside from that, being a Senior Support Practitioner isn’t all about qualifications, if you have a passion for making a difference then we can provide you will all the training and support that you need. You can also take us up on our offer to support you through your NVQ in Health and Social Care qualification!Your role will be working on a Permanent contract working full time, 35 hours per week across our rota basis.
